When you're going out simply enter the number and phone number of where you can be reached, your estimated return time, and any special instructions. All About Me! contains all the vital information your sitter needs should an emergency arise including: a "permission to treat" form for emergency medical care; your address and phone number; the phone numbers of the nearest hospital, your pediatrician, police, fire, and the poison control center; and the names and phone numbers of other emergency contacts. It also provides a complete medical history, including immunizations, surgeries, medical problems, medications, allergies, etc. Simply click the print button to print a form containing all this information for your babysitter.
