If you already have a programmable joystick you can still use Controller Command Module because it lets you program keyboard keys to act just like a joystick button.
You can also program more buttons than you really have. Controller Command Module can be programmed with up to five modes, with each mode capable of being programmed independently. So, if your joystick has eight buttons and you use five modes, you suddenly have 40 buttons to program with whatever you want.
